New Patient Information

The physicians at Consultants In Cardiology provide a wide range of services including consultation and management of hypertension, coronary artery disease, valvular heart disease, cardiac arrhythmias, pacemaker / defibrillator placement, vascular disease, and preventive cardiology / lipid management. We also perform diagnostic testing with state of the art equipment in a professional and friendly environment at the request of referring physicians. We welcome you to our practice and look forward to serving your medical needs.

To assist our new patients with their first appointment, we offer the following helpful hints:
  • Prior to your appointment, please obtain and forward copies of your medical records.
  • Please arrive 15 minutes prior to your scheduled appointment.
  • While you will be pre-registered over the phone, it will still be necessary to complete paperwork at the time of your appointment, so please remember to bring your insurance card and picture I.D.
  • You will be required to sign a HIPAA Privacy Act Form at the time of your visit. If you have any questions regarding this form, please do not hesitate to ask.
  • It is recommended you write down any questions you may have for the physician in preparation for your visit.
  • Please arrive promptly at the time of your scheduled appointment and plan on being at the office for approximately one hour.
  • Please bring all of your medications, including over-the-counter remedies, with you for your appointment.
  • If you have a HMO insurance, you are responsible for obtaining the necessary referral. If your referral is not available at the time of your visit, your appointment may need to be rescheduled.
  • If you have a co-payment, BCBS Master Medical, or no insurance, you will be responsible for payment at the completion of each visit. We accept Visa, Mastercard, check, or cash. If you have further billing questions, please refer to our Billing Site.
  • When calling Consultants in Cardiology, please listen to the automated attendant carefully. You can enter your option at any time during the message and you will be promptly transferred to the appropriate person. If you receive a voice mail, please leave a message including name and phone number. At Consultants in Cardiology we strive to return all phone calls by the end of the day. Our office hours are 8:00 am to 4:30 pm, Monday through Friday.
